Aim of the Service

All children who are looked after (that means social services have some responsibility for your care) must legally have a health examination when they first become looked after and then again every year after that.

You will be weighed and measured and the rest of the assessment may well only be a chat and getting to know you.
You can bring a parent/carer with you or you may wish to be seen on your own – this is for you to decide.
If any of the information given needs to be shared with any other agency you will be told of this so that you can give your permission.
You will be given the chance to talk about any health problems or issues you have i.e. skin problems, smoking.

If it is your first Health Check, it will be a Paediatrician (a special doctor for young people) and a special nurse who works with looked after children – she won’t wear a uniform.
If it is not your first check then it could be a health visitor (if your aged under 5!) or your school nurse. If you prefer you can ask janet to carry out the health check whatever age you are!

The place and date of the medical assessment will be sent to you via your carer, but if you would prefer it to be done somewhere else (within reason) or on a different day you can phone this number 01603 437811
If the assessment shows you need to be referred to someone else or you need to have some treatment this will be organised.
You will also have a chance to ask any questions you have and get any health advice you want (so go prepared).
This assessment has to happen annually so afterwards let us know what we can do to make it easier.

So the aim for the service is:-
To improve the health of children looked after.
The objectives are:-
To co-ordinate the child or young person’s health records.
Ensure the views of children and young people are listened to, and to involve them in decisions about their health.
To promote healthy lifestyles for young people by giving them information and encouraging them to attend relevant clinics and services.
To provide support and advice for carers.
To contribute to on-going training and education in health matters for professionals and users of the service.
